This position works collaboratively with providers and other members of the health care team to improve the health of patients with chronic disease conditions and/or complex needs; educates patients and families to help them manage their health care needs; facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management. Education - Completion of a BSN, or an ADN from an accredited institution.Experience - Two years ambulatory nursing experience with BSN or three years with an ADN preferred, in areas such as public health, community health, or equivalent and experience in multicultural nursing or other social service area and commitment to community health care preferred. Care management or chronic disease management experience preferred.
