Program: Institutional Special Needs Program (ISNP)Location: Grant, Madison, and Delaware Counties, INSchedule: Full-time (40 hours/week), Monday–Friday, 8:00 AM – 5:00 PMDuration: Approximately 7 monthsTravel: Required between 7 facilities during shiftDress Code: Medical attire (no specific scrub color required)About The RoleJoin our client's Senior Community Care (SCC) team, a leading integrated care delivery program focused on high-risk populations in long-term care settings. As an Advanced Practice Clinician (APC), you’ll be part of an interdisciplinary team delivering personalized, proactive care to improve outcomes and quality of life for our members.This role offers work-life balance with no on-call, weekend, or holiday requirements, and flexible scheduling options.Key ResponsibilitiesPerform comprehensive clinical assessments and manage acute and chronic medical and behavioral conditions.Document diagnoses and care plans accurately, including ICD-10 coding.Collaborate with interdisciplinary care teams to coordinate patient care.Lead advanced illness and care planning conversations with patients and families.Provide education and counseling to patients and caregivers.Basic QualificationsActive and unrestricted NP or PA license in the state of practice (or ability to obtain by start date).Graduate of an accredited NP (MSN or DNP) or PA program.Board certification:NP: AANP or ANCC in Family, Adult, Gerontology, or Adult-Gerontology Acute Care.PA: NCCPA certification.Active DEA license and prescriptive authority.Ability to lift 30 lbs, navigate stairs, and perform physical assessments in varied environments.Reliable transportation and ability to travel between facilities.Ability to obtain a collaborative practice agreement (if required by state).Fully vaccinated against COVID-19 (per federal, state, and client requirements).Preferred Qualifications1+ year of clinical experience (long-term care preferred).Experience managing patients with complex behavioral, social, or functional needs.Knowledge of geriatrics, chronic illness, and end-of-life care.Proficiency in electronic medical records and clinical documentation.By providing your phone number, you consent to: (1) receive automated text messages and calls from the Judge Group, Inc. and its affiliates (collectively "Judge") to such phone number regarding job opportunities, your job application, and for other related purposes.
