Pelican Pointe Post Acute Nursing & Rehabilitation - Responsibilities:Assessment:Conducting thorough evaluations of residents' physical condition, including range of motion, strength, balance, and pain levels.Treatment Planning: Developing individualized treatment plans based on the assessment, incorporating exercises, manual therapy, and other therapeutic interventions.Implementation: Guiding residents through exercises, stretches, and other therapeutic activities to improve mobility, strength, and function.Equipment Training: Instructing residents on the proper use of assistive devices like walkers, wheelchairs, and canes.Monitoring and Adjustment: Regularly assessing residents' progress and making necessary adjustments to treatment plans.Education: Educating residents and their families on strategies for fall prevention, pain management, and home exercise programs.Collaboration: Working closely with nurses, doctors, and other healthcare professionals to ensure comprehensive care.Documentation: Maintaining accurate and up-to-date records of resident assessments, treatment plans, and progress.Falls Prevention: Implementing strategies to reduce the risk of falls, such as balance training and environmental modifications.Pain Management: Utilizing various techniques to help residents manage pain and improve their comfort levels.Promoting Independence: Empowering residents to regain or maintain their independence in daily activities such as dressing, bathing, and eating.
