Outreach & Engagement Manager - Community Support Program

The Outreach & Engagement Manager leads a team across Manhattan to connect disconnected and homeless individuals with care by collaborating with referral sources, such as hospitals, shelters, and social service organizations, and canvassing subways and streets. They evaluate referrals for service eligibility, act as the primary liaison for ensuring timely enrollment, and manage community outreach efforts. This role involves coordinating participants' needs, enhancing their daily living skills, and advocating on their behalf.
Essential Position Functions:
Lead a team to connect disconnected and homeless individuals with care across Manhattan, working with referral sources and canvassing subways and streets.
Evaluate referrals from community partners, SOS HUB, and OMH to determine service eligibility.
Act as the primary liaison for referrals, ensuring timely enrollment and addressing all referrals with staff.
Conduct community outreach, coordinate participant needs during transitions, enhance daily living skills, accompany participants to appointments, and advocate for them.
Build connections with community-based providers to ensure participants access all eligible services.
Complete and submit all referral tracking documents to Directors and CBC.
Provide accurate, complete, and timely documentation.
Collaborate with Directors to manage staff scheduling, task completion, and fulfill job requirements.
Required Knowledge, Skills, and Abilities:
Bachelors Degree in a related field.
Work Schedule: Saturday to Wednesday, 9 AM - 5 PM
Preferred experience with homeless or precariously housed populations.
Experience working with individuals with a behavioral health diagnosis preferred, but not required.
Proficient in Health Information Technology and Microsoft applications (Word, Excel, PowerPoint).
Spanish-speaking ability is a plus.
