LPN - Licensed Practical Nurse

*Job Overview* 1st shift 0600-1400 - but MUST be flexible to work 2nd shift 13:30-21:30pm and available to work any day to including weekend/holidays. Must be flexible as schedule/shift/days can change depending on facilities needs Candidate must be available to start and have no upcoming scheduling issues coming up (vacations, appointments, school, etc) - be on time to work each day and on the floor at the time their schedule starts - example - if shift starts at 7am - they must be on the floor at 7am ready to work (not coming thru security at 7am). *Responsibilities* * Perform comprehensive patient assessments including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. * Administer medications, injections—including IV infusions—and treatments following physician orders while ensuring adherence to HIPAA and clinical confidentiality policies. * Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing techniques. * Monitor patients’ conditions closely, manage acute pain, and provide triage assessments to prioritize urgent needs in emergency or urgent care environments. * Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with diverse needs such as dementia, Alzheimer's disease, developmental disabilities, or post-acute recovery. * Support discharge planning and case management efforts by educating patients and families on medication administration, mobility aids like Hoyer lifts, and home & community care strategies. * Maintain accurate medical documentation including ICD coding (ICD-9/ICD-10), medical records management, and detailed clinical notes to ensure compliance with healthcare regulations. *Skills* * Proven experience working with electronic medical records (EMR/EHR systems) such as Epic, Cerner, Athenahealth or eClinicalWorks. * Strong background in critical care environments including ICU, emergency room (ER), trauma centers (Level I & II), or hospital medicine settings. * Expertise in specialized areas such as geriatrics, memory care (including Alzheimer’s and dementia care), hospice care, infectious disease management, and pain management. * Proficiency in performing procedures like venipuncture (phlebotomy), IV insertion/infusion therapy, sterile technique, airway management including ventilator support, and specimen collection/processing. * Knowledge of long-term care regulations and experience working with geriatric patients or individuals with disabilities across assisted living facilities or skilled nursing facilities. * Ability to communicate effectively with patients of all ages—including children—and their families while demonstrating compassionate caregiver communication skills. * Familiarity with medical terminology related to internal medicine, pediatrics, neonatal ventilators, spinal taps, gastric lavage, and other specialized treatments.
