Clinical Social Worker

POSITION SUMMARY:Supports and coordinates the social service function for patients and their families, providing discharge planning pertaining to illness, hospitalization and financial concerns and facilitates post discharge planning. Works in collaboration with the Case Manager to identifies potential barriers to patient throughput , and quality outcomes. Works within the structure to negotiate and resolve problems and conflicts during the patient stay to ensure continuity of care.QUALIFICATIONS:Master's degree in Social Work or Counseling Psychology requiredOne of the following Florida licenses is required:Licensed Clinical Social Worker (LCSW)Licensed Mental Health Counselor (LMHC)One year experience in discharge planning in a health care setting requiredBasic Life Support (BLS) requiredMust have experience - must have FL LCSW. Needs knowledge of FL Baker Act Law, prior experience in discharge planning/case management/facilitating groups.Needs strong communication skills. Will be working closely with physicians/nurses/UR department and attending treatment team daily.
