Field Nurse Practitioner
This Nurse Practitioner role provides primary care and preventive services in non-traditional settings such asin-home, community, nursing facilities, and pop-up clinics, aiming to improve member health outcomescost-effectively. Responsibilities include conducting comprehensive evaluations, managingchronic and acute conditions, coordinating care, ordering diagnostics, and developing treatmentplans. The role involves extensive regional travel (up to 120 miles/day), participating incommunity outreach, and providing post-discharge support to reduce hospital readmissions.Candidates must have at least 1 year of experience as a nurse practitioner, active state license, DEAregistration, and certification from AANP or ANCC. The position requires strong communication,problem-solving skills, and the ability to work independently across diverse settings. Preferredqualifications include experience in home or community health and bilingual skills.
