Job ID: HOP Case Manager Location: TBD Full/Part Time: Full-time Regular/Temporary: Regular Job Summary: Our agency is seeking a Case Manager with a well rounded human services background to provide comprehensive non-medical interventions and resources related to housing, food, transportation, interpersonal violence and toxic stress in New Hanover County. Staff will address the conditions in which people live that directly impact health outcomes. Requires extensive knowledge in case management, housing programs, documentation, and vast awareness of community resources. Our agency must be represented in a friendly and professional manner. Experience with working with formerly incarcerated or marginalized populations is beneficial; lived experience with mental health conditions, substance abuse, or trauma. Must have the ability to be on-call for emergencies and work on weekends and public holidays for pre-crisis and post crisis situations. Duties and Responsibilities: Under general supervision, the Case Manager will report to the Reentry Director as the frontline social service provider that communicates with the Network Lead representatives and deliver authorized, pilot services to clients. Identify resources to support the needs of clients. *Bolded bullet points are high priority essential job functions.* ● Ability to work flexible hours, including evenings and weekends and some holidays on occasion to serve client’s needs; responding to call-outs and client emergencies as needed. ● Assisting in facilitation of orientations-individual or group; assist clients/resident support groups with assigned demographics on a regular basis; as well as means of locating and joining existing groups. ● Assisting clients by finding resources, advising the clients of processes, and encouraging follow through with proposed resolutions, locating social activities, or providing other assistance as needed. ● Assist with conduction of interviews of incoming clients or process referrals to determine their needs/goals and develop an individualized plans; may include assessment of need to secure public assistance, social services, or education needs for post release plans (such as literacy, behavioral health, and housing providers). ● Attends agency related meetings, conferences, and programs. ● Coordinate with care management teams on the delivery of pilot service to clients. ● Collecting data ( record maintenance & quality assurance) and other information that contributes to wellness and recovery; generating recovery review and input data in appropriate database(s); document all interactions in NC Care 360 portal. ● Establish and maintain a trusting and supportive relationship with clients and teammates. ● Fostering client’s development of healthy habits and healthy relationships by encouraging participation in community activities; modeling effective coping and self-help techniques to clients. ● Interact with NC CARE 360 platform to connect enrollees to community resources and track whether the client accessed the resource. ● Initiate, establish and promote strong relationships with key stakeholders across healthcare, community-based organizations, social services, and other human service organizations. ● Maintain client confidentiality at all times and in accordance with LINC, Inc. policies and procedures; maintain an accurate and organized schedule in shareable electronic calendar. ● Meets frequently with Reentry Director, Case Managers, Community Health Worker, Peer Support Specialist and other appropriate staff as required; inform of case acceptance and completion ratios for significant treatment. ● Meeting with clients regularly offering one-on-one support to provide advice and counseling in accordance with their treatment plan; teach clients how to identify negative self-talk and overcome fears with person-centered strengths, supplemental aids, or resources (literature, restorative media, etc.); monitor the client’s behavior, schedule, medication, and activities. ● Represent the agency with excellent customer service at all times. ● Reporting any unusual behavior, conflicts, or recovery deviations to the relevant professionals (i.e., Case Managers, Community Health Worker, Peer Support Specialist, Program Supervisors, Executive Director, etc). ● Profound knowledge of basic first aid and CPR techniques and emergency procedures. ● Provide locally based, high-quality services that address client needs related to housing instability, transportation insecurity, food insecurity, interpersonal violence and toxic stress. ● Performs other related duties as required. Physical Demands: The physical demands described are representative of physical aspects of this position classification must perform in carrying out essential job functions. Work is performed primarily in an office setting but may include some remote or virtual. Some local travel required. ● Persons performing service in this position classification may exert up to 50-75 pounds of force frequently to lift, carry, push, pull or otherwise move objects. ● This type of work may involve ascending and descending stairs and ramps, and will involve walking or standing and sitting for extended periods. ● Perceiving the nature of sound, near and far vision, depth perception, providing oral information, the manual dexterity to operate equipment and handle or work with various materials, tools, or objects are important aspects of this role. ● Exposure to hot, cold, wet, humid, or windy conditions caused by weather may occur occasionally. Minimum Requirements: Highschool diploma or equivalent (GED certificate) or Associate’s Degree in a Human Services field. One (1) year experience providing case management and/or treatment services in similar settings; or an equivalent combination of training and experience sufficient to successfully perform the essential duties of the job such as those listed above. An equivalent combination of education and/or experience may be substituted if it directly relates to the position's major responsibilities and duties. Communication and customer service skills. Excellent time-management abilities. Preferred Requirements: Bachelor’s Degree in Psychology, Social Work, or equivalent or Associate’s Degree in a Human Services field. Two to five (2-5) years of work experience in working in culturally sensitive environments and/or workforce program settings. Previous experience with the Healthy Opportunities Pilot program, Unite Us and/or NC CARE 360 portal; vast familiarity with non-medical interventions and resources related to housing, food, transportation, interpersonal violence and toxic stress in New Hanover County; and knowledge of SDOH. Communication and customer service skills; reflective listening skills or listening in a manner to identify needs and re-phrasing of information heard for clarification.
