Clinical Coordinator - Safe Haven

Essential Position Functions:
Participate in individual supervision with Area Director and Program Directors for operational and clinical oversight.
Assist in supervising Case Management staff, offering guidance on complex clinical cases and admissions, and contributing to psychiatric evaluations for housing applications.
Monitor medication compliance through spot-checking documentation and liaising with prescribers and pharmacies.
Manage a caseload focused on high-risk clients, ensuring comprehensive care and updated treatment plans.
Collaborate with clinical teams and specialized staff to develop and update client treatment plans, coordinate care with internal and external providers, and participate in incident reviews and DHS Safe Haven meetings.
Attend weekly team meetings and client case conferences as required, offering consultation on complex cases.
Provide direct care services during emergencies, offering crisis intervention, counseling, and psychosocial assessments. Act as a liaison between staff, hospitals, and clinics regarding client treatment plans.
Maintain detailed documentation in electronic medical and case records systems (AWARDS and CARES).
Support the program in preparing for site visits, inspections, and audits, ensuring compliance with regulatory standards and best practices.
Required Knowledge, Skills, and Abilities:
A Master’s degree in social work or mental health counseling with relevant licensure is preferred.
Experience in working with individuals facing substance abuse and mental health issues.Strong computer literacy required; proficiency in using Electronic Health Record (EHR) systems to document services.
Knowledge of AWARDS, CARES, and BCS systems is advantageous.
Potential for clinical supervision to accumulate LCSW/LMHC hours, contingent on staff availability.
Must possess a clean driver’s license; ability to work at multiple locations and access to agency vehicles provided as needed.
Provided with an unlimited monthly MetroCard for commuting needs.
Bilingual proficiency in Spanish is a plus.
