Case Manager 164 Part Time - $28,919 - Edison/Old Bridge, NJ Salary - $28,919 per year Responsibilities: Provide case management services to individuals and families residing in Permanent Supportive Housing sites including completion of intakes, developing service plans, linkages to other services including, conflict de-escalation, managing relationships with a broad range of community providers, promoting housing retention through teaching life skills, setting up programming for tenants. Utilization of HMIS case management software. Requirements: Bachelor’s degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred. Have a familiarity with housing first, progressive engagement, and reality testing. Experience with conflict resolution and mediation. (2) years relevant work experience and demonstrated ability to provide effective services to clients. Needs to be culturally competent across all populations. Bilingual a plus (Spanish). Ability to work independently. May include additional on-call hours with evening hours, flexibility in creating own work schedule. Valid driver’s license with acceptable driving history.
