EOE Oakridge Homes is currently looking for a Designated Coordinator who: · Meets the qualifications of a DC in education and work experience by providing direct care services to persons with disabilities or persons aged 65 or older as follows: a. a baccalaureate degree in a human services related field, and 1 year of full-time work. b. an associate degree in a human services related field, and 2 years of full-time work. c. a diploma in a human services related field from an accredited postsecondary institution and 3 years of full time work; or d. a minimum of 50 hours of education and training related to human services and disabilities; and 4 years of full time work under the supervision of a staff person who meets the qualifications identified in any of the first three described above. · Has excellent verbal/written communication skills · Will office based out of Clarissa, MN with work and day travel primarily in Todd and Wadena counties and the surrounding area · Can provide a valid MN driver’s license and clean driving record · Can pass a DHS background study As a Designated Coordinator, you will: · Perform highly responsible duties related to the development, implementation and coordination of Individual Service Plans for persons served with developmental disabilities or mental health needs · Maintain, review and evaluate pertinent program data related to each individual’s growth and development · Ensure that services are being correctly implemented · Provide training to direct care staff as indicated Oakridge Homes offers: · Full Time-40 hours per week · Comprehensive benefits package including Medical and other options · Competitive wages-Starting at $21 per hour · Paid training and meetings EOE Job Type: Full-time Pay: From $21.00 per hour Benefits: 401(k) 401(k) matching Dental insurance Employee assistance program Flexible spending account Health insurance Life insurance Paid time off Vision insurance Schedule: 10 hour shift 8 hour shift Day shift Evening shift Weekends as needed Work Location: In person
