DescriptionKings View is a nonprofit leader in providing behavioral health services to the underserved community, is currently seeking a driven Case Manager II to join our mission to provide compassionate services to those with limited resources. Our ideal candidate will have a passion to promote social justice and societal awareness, respect for human dignity, and lead with integrity while listening and responding to the community in need.Case Manager II (597)Hanford, CAProvide rehabilitation services including individual and group services to consumers receiving service in the Adult Outpatient program. Provide linkage and support to consumers in the Adult Services. Assist with the coordination of all pertinent treatment information and communicate between the different treatment team members in various treatment team meetings, etc.How will I contributeComplete all documentation of services provided to or on behalf of consumers; as set forth in Kings View's policy and procedure manual.Participate in continued education, especially in dialectic behavioral training and wellness and recovery model/concept.Interface with public and private agencies (such as Casa Del Rio, Plumlee's, Ruby's and BHA) as necessary in the course of conducting skills training services. These agencies may include; Child Protective Services, Probation, Public Guardian Office, and other agencies as required.Contribute creative ideas to make program more effective.Conduct home evaluations and observations when needed to collect all necessary and pertinent information or history to develop appropriate treatment plans, rehabilitative goals with consumers.Maintain a yearly productivity standard of 1200 billable hours. It is the responsibility of the case manager to seek out referrals from ASOC therapists.There may include some evening hours based on program need.Achieve basic competence in using our electronic record keeping system: a. Effective use of Anaszai Scheduler; b. Effective use of Clinician Home Page; c. Effective creation of progress notes; d. Effective completion of Service Activity Logs (SAL)Complete documentation and SAL on same day service at a rate of 80% or better.Commit to learn DBT, and other related interventions as needed.Achieve all performance standards.Complete four hours of cultural competency annually.Familiarity with and adherence to compliance with the code of conduct and the requirements of the compliance program are elements of performance evaluations and conditions of employment.In the course of performing job duties, follows all Kings View policies and procedures, administrative directives, all relevant county, state and federal regulations, and any performance obligations contained in Kings View program contract with other entities.Other duties as assigned What I bring A. degree plus 4 years experience, or LPT/LVN/RN plus 4 years experience, or B.A. degree plus 2 years experience, or 4-year degree in activity therapy or related field from a college or university plus registration or certification with the respective professional organization and 2 years experienceClass C Driving LicenseExpertise in program services delivery - practical and theoretical.Data entry into various electronic systems while maintaining the integrity and accuracy of the data. Relate to others comfortably and able to establish and maintain effective relationships.
