Program Manager
POSITION SUMMARY
We are seeking a responsible, organized, and experienced *Program Manager* to oversee the daily operations of a licensed 245D Community Residential Services (CRS) program.
The Program Manager is responsible for ensuring that the program operates in accordance with *Minnesota 245D requirements, licensing standards, client rights, individual service plans, company policies, and applicable DHS regulations*.
The Program Manager will provide leadership to Direct Support Professionals (DSPs), coordinate services, maintain required documentation, monitor staff performance, support individuals receiving services, and ensure the home remains safe, compliant, and person-centered.
The ideal candidate is dependable, professional, organized, a strong communicator, and able to work independently while maintaining high standards of care and compliance.
ESSENTIAL RESPONSIBILITIES
1. Program Operations
* Oversee the daily operations of the licensed 245D group home.
* Ensure services are delivered according to each person's Individual Service Plan (ISP), CSSP, Addendums, and other applicable service documents.
* Ensure the home maintains a safe, clean, respectful, and person-centered environment.
* Monitor household routines, appointments, activities, transportation, meals, medications, and other program responsibilities.
* Ensure adequate staffing coverage and respond promptly to staffing concerns.
* Communicate program concerns and operational needs to the Program Director.
2. 245D Compliance & Licensing
* Maintain compliance with applicable Minnesota 245D requirements and DHS licensing standards.
* Ensure required program documentation is completed accurately and on time.
* Monitor staff compliance with required training, orientation, policies, procedures, and job responsibilities.
* Maintain required personnel and program records.
* Assist with licensing reviews, corrective action plans, and preparation for licensing visits.
* Identify compliance concerns and take appropriate corrective action.
* Ensure client rights are protected and respected at all times.
* Ensure prohibited practices, maltreatment, neglect, and other violations are promptly addressed and reported according to applicable requirements.
3. Staff Supervision
* Supervise and provide direction to Direct Support Professionals.
* Conduct regular staff meetings and individual supervision as required.
* Assist with hiring, onboarding, orientation, and training of new employees.
* Monitor staff attendance, punctuality, professionalism, and job performance.
* Provide coaching, feedback, and corrective action when necessary.
* Ensure staff follow individual service plans and company policies.
* Address staff conflicts and performance concerns professionally and promptly.
* Maintain appropriate staffing schedules and arrange coverage when needed.
* Ensure staff maintain professional boundaries with individuals served.
4. Client Support & Person-Centered Services
* Promote the dignity, independence, choice, privacy, and rights of each person served.
* Ensure services are delivered in a person-centered manner.
* Monitor progress toward individual goals and outcomes.
* Communicate with individuals served, guardians, case managers, healthcare professionals, and other members of the support team as appropriate.
* Assist with identifying changes in an individual's needs and communicating those changes to the appropriate parties.
* Ensure individuals have opportunities to participate in meaningful activities in the home and community.
* Support individuals in developing independence and maintaining community connections.
5. Documentation & Recordkeeping
* Review daily documentation and ensure required records are complete and accurate.
* Monitor progress notes, service documentation, medication records, incident reports, health and safety documentation, and other required records.
* Ensure documentation is completed within required timelines.
* Correct documentation deficiencies and follow up with staff when records are incomplete.
* Maintain confidentiality of all client and employee information.
* Ensure records are organized and readily available for licensing and program review.
6. Incident & Emergency Response
* Respond appropriately to emergencies, accidents, behavioral incidents, medical concerns, and other critical situations.
* Ensure required incident reports are completed accurately and timely.
* Follow company policies and applicable reporting requirements.
* Notify appropriate supervisors, emergency personnel, guardians, case managers, and other parties when required.
* Participate in follow-up and corrective action after incidents.
* Ensure staff understand emergency procedures and reporting expectations.
7. Medication & Health/Safety Oversight
* Ensure staff follow applicable medication administration and documentation procedures.
* Monitor medication-related documentation and report concerns appropriately.
* Ensure medical appointments and healthcare recommendations are followed as required.
* Monitor health and safety risks within the home.
* Ensure emergency plans and health-related protocols are followed.
* Promote a safe environment for individuals and staff.
8. Communication
* Maintain professional communication with staff, supervisors, individuals served, families/guardians, case managers, healthcare providers, and other professionals.
* Communicate changes in client needs, staffing concerns, incidents, and program issues promptly.
* Participate in meetings related to individuals served and program operations.
* Maintain professional boundaries and confidentiality.
QUALIFICATIONS
Required
* Must meet applicable Minnesota 245D requirements for the position.
* Experience working with individuals receiving residential or disability services preferred.
* Strong understanding of person-centered service delivery.
* Strong organizational and communication skills.
* Ability to supervise and provide direction to staff.
* Ability to maintain accurate and timely documentation.
* Ability to handle confidential information appropriately.
* Ability to respond effectively to emergencies and unexpected situations.
* Ability to work independently and make sound decisions.
* Valid driver's license and reliable transportation, if required by the program.
Preferred
* Previous experience as a 245D Program Manager, Designated Manager, Supervisor, or similar leadership position.
* Knowledge of Minnesota DHS licensing requirements.
* Experience with Community Residential Services (CRS).
* Experience supervising Direct Support Professionals.
* Experience working with individuals with disabilities and/or complex support needs.
* Experience preparing for DHS licensing reviews.
REQUIRED TRAINING & BACKGROUND CHECKS
Employment is contingent upon completion of applicable requirements, which may include:
* Background study in accordance with Minnesota requirements.
* Required 245D orientation and training.
* First Aid/CPR certification.
* Medication administration training, when applicable.
* Positive support and/or behavior-related training, when applicable.
* Annual and ongoing training required by the organization and applicable regulations.
