Cannot have any major moving violations in the last yearCannot have been fired for safety reasons in the last 6 months and cannot have been fired from last job for safety reasonsNo key preventable CMV accidents in the last five years (such as rollover, rear end, etc) — however if driver has had 6 months of safe driving in the last year it will review if only accident in last five yearsNo more than one minor preventable accident in a CMV on a roadway in the last 2 yearsNo more than three CMV accidents in a private roadway or parking lotNo DUIs in the last ten years (if have held Class A CDL less than five years cannot have any DUIs in the last five years)No more than one DUI ever and can never have one in a CMVNo failed DOT drug or alcohol tests ever — we do NOT TAKE SAPJ-18808-Ljbffr
