Join us in our mission to enhance the well-being of our patients and residents while making a meaningful impact in the communities we serve.ResponsibilitiesEnrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs. The goal is to provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and compliance with federal, state, and local guidelines and regulations, company policies and procedures, and standards of care.*Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
