The Licensed Practical Nurse isresponsible for providing direct patient care underthe supervision of the Registered Nurse. Responsibilities include following the plan of care, providingtreatments, and working collaboratively with the members of the team to help meet positive patientoutcomes.Essential Job Functions* Provides direct patient care within the parameters of current nursing license.* Implements current nursing practice by following the patient assessment and approved plan ofcare, which was initiated by the Primary Registered Nurse.* Complete, maintain and submit accurate and relevant clinical notes regarding patient's condition andcare. Record pain and symptom management changes and/or outcomes as appropriate.* Point of Service documentation is an expectation of all clinical personnel for quality care,accuracy, and communication standards. The clinician is expected to appropriately document at Pointof Service, and effectively communicate with the Patient Care Team.* Provide patient and family/caregiver education and information pertinent to diagnosis and self plan ofcare.* Participates in the coordination of Home Health services, appropriately reporting the identified needsto RNUses and prepares equipment and supplies effectively, adhering to aseptic technique.Participates in personal and professional growth and development.Participates in the organization's quality management program.Other duties as assigned.Qualifications(Minimum qualifications will be considered required unless specifically stated otherwise)Experience:* Minimum of one (1) to two (2) year's experience in health care. Three (3) years experience in healthcare preferred.Experience in a home health care preferred.Experience in acute care in an institutional setting preferred.Experience in homecare preferred.Education and Training:(Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination of these. May beexpressed in terms of formal educational equivalents):* Graduate of an accredited Practical Nurse or Vocational Nursing program.License, Certification & Registration:Possesses and maintains current CPR Certification.Current Massachusetts licensure: LPN/LVNValid driver's license and automobile insuranceOther Requirements:* Demonstrates good verbal and written communication, and organization skills.* Strong observation skills and problem solving skills.Working Conditions* Uniform/ Dress Requirements (Beyond those requirements outlined within Policy No. C:3-015.1):NoneWorking Conditions:* May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weatherconditions on roads in varying repair.* May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances.May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.Handles emergency or crisis situations. Travel required.
