Job Description Home Care Coordinator – All Connecticut Home Care Services All Connecticut Home Care Services is seeking a compassionate, motivated, and organized Home Care Coordinator to join our fast-growing team. This position is ideal for someone with strong communication and organizational skills and a genuine passion for serving the disabled and elderly communities. The ideal candidate will have experience with client intake, staff recruitment and supervision, and coordination of care, along with a working knowledge of Medicaid and private-pay systems. Prior experience working in a homemaker and companion care setting is required. Serious inquiries only. Key Responsibilities Serve as the primary point of contact for Medicaid referrals and client intake. Possess a minimum of two (2) years of experience working within a homemaker and companion agency. Demonstrate familiarity with community-based Medicaid programs , including referrals from community care programs and the Agency on Aging . Support marketing and business development efforts to grow the private client base, including outreach to retirement communities, veterans' homes, and other referral sources. Coordinate, maintain, and manage client files for both Medicaid and private-pay clients in full compliance with agency policies and state regulations. Recruit, interview, and hire qualified home health aides, including Certified Nursing Assistants (CNAs) and Personal Care Assistants (PCAs) . Supervise caregivers to ensure high standards of client care and satisfaction. Monitor staffing needs and conduct required background checks for prospective employees. Maintain accurate and up-to-date electronic records. Conduct client outreach and follow-up to ensure ongoing satisfaction with services provided.
