This role involves coordinating with Case Management, nursing, physicians, and multidisciplinary teams to support safe and efficient patient discharges, participating in daily Multidisciplinary Rounds and virtual afternoon bed huddles to identify and resolve discharge barriers, and ensuring smooth transitions of care. The position offers excellent hours and broad clinical exposure. Prior APP experience is required, with strong preference for candidates with internal medicine experience. New graduates will not be considered. BLS and DEA certification are required.
