RN MDS Coodinator

River Ridge is seeking an experienced RN MDS Coordinator to oversee the completion and management of MDS assessments in compliance with CMS and New York State regulations. This role is essential to accurate clinical documentation, care planning, and reimbursement. Key Responsibilities Complete and submit MDS assessments (OBRA, PPS, Significant Change) accurately and on time Coordinate with the interdisciplinary team to develop and update care plans. Ensure compliance with MDS 3.0, RAI process, PDPM, and NYS DOH requirements. Monitor Medicare/Medicaid reimbursement and case mix accuracy. Conduct chart audits and provide staff education as needed Support survey readiness and quality initiatives Qualifications Active New York State RN license (required) MDS Coordinator experience in a skilled nursing or long-term care setting (preferred) Strong knowledge of MDS 3.0, PDPM, and CMS regulations Excellent organizational and communication skills EMR experience required What We Offer Competitive pay Health, dental, and vision insurance PTO and holidays Supportive leadership and team environment
