Job Description Position SummaryWe are seeking a skilled and compassionate Registered Nurse (RN)/ Licensed Practical Nurse to provide care in a home healthcare agency. The Field Nurse is responsible for delivering high-quality patient care, conducting assessments, coordinating services, and ensuring compliance with all regulatory standards.Key ResponsibilitiesPerform comprehensive patient assessments, including OASIS documentation - RN onlyDevelop, implement, and update individualized care plans - RN onlyProvide skilled nursing care and monitor patient conditionCoordinate care with physicians, caregivers, and interdisciplinary teamsEducate patients and families on care plans, treatments, and disease managementComplete timely and accurate clinical documentationSupervise and evaluate home health aides and LPNsParticipate in case conferences and care coordination activitiesEnsure compliance with Medicare, Medicaid, and regulatory requirementsQalificationsActive Registered Nurse (RN) license in the applicable stateGraduate of an accredited school of nursingMinimum 1 year of clinical experience (home health preferred)Knowledge of Medicare/Medicaid guidelines and home health regulationsStrong assessment, documentation, and communication skillsCurrent CPR certificationValid driver's license and reliable transportationAbility to work independently and manage time effectivelyWilling to travel to clients' homes to provide careWork Environment & RequirementsTravel to patient homes requiredAbility to perform physical tasks including standing, walking, and assisting patientsExposure to clinical and home care environmentsWhy Join UsMeaningful, patient-centered workFlexible scheduling opportunitiesCollaborative clinical support environment
