Based in San Ramon, we serve patients throughout the surrounding communities and foster a collaborative, supportive culture for our clinical team.We are currently seeking a licensed Physical Therapist (PT) to join our interdisciplinary home health team. This role offers professional autonomy, strong clinical support, and the opportunity to make a meaningful impact on patients’ mobility, independence, and quality of life.Position OverviewAs a Physical Therapist with Qualicare Home Health, you will provide skilled in-home physical therapy services, conduct comprehensive evaluations, develop individualized treatment plans, and collaborate closely with caregivers and clinical team members to deliver coordinated, outcomes-focused care.Key ResponsibilitiesConduct initial evaluations, re-evaluations, and discharge assessments in accordance with Medicare and agency standardsDevelop and implement individualized treatment plans aligned with patient goals and physician ordersProvide therapeutic interventions including gait training, balance activities, strengthening, range-of-motion exercises, and fall preventionEducate patients and caregivers on safety, home exercise programs, and proper use of assistive devicesCollaborate with PTAs, RNs, OTs, and other interdisciplinary team members to ensure continuity and quality of careAccurately complete clinical documentation using OASIS and the agency’s EMR system in a timely mannerMonitor patient progress, identify changes in condition, and communicate effectively with the care teamSupervise Physical Therapy Assistants (PTAs) and aides in compliance with state and federal regulationsQualificationsActive and unrestricted California Physical Therapist (PT) licenseMinimum 1 year of home health experience, including OASIS documentation (required)Experience with EMR systems; WellSky/Kinnser preferredStrong clinical judgment, organizational, and communication skillsAbility to work independently while collaborating with an interdisciplinary teamValid California driver’s license and reliable transportationCompensation & BenefitsAnnual salary: up to $150,000, **commensurate with experienceMedical, dental, and vision insurance401(k) with employer matchPaid time off (PTO) and paid holidaysCEU and license renewal reimbursementFlexible scheduling and caseload autonomyWeekly team check-ins and strong clinical/field supportH-1B visa sponsorship available for qualified candidatesJoin Our TeamAt Qualicare Home Health, we value clinical excellence, professional respect, and work-life balance.
