Join Our Dynamic Case Management Team Job Description: - Exciting opportunity for an MSW professional to support the Case Management Department! - Work Schedule: Monday to Friday, 7:30 AM - 4:00 PM (hours may flex to 8:00 AM - 4:30 PM). - Serve as a vital resource to assist with: - Insurance authorizations - Post-acute referrals - Delivering Medicaid notices to patients - Clinical background in acute care is helpful but not required! - Experience working with Epic charting system is a must! - Average case load: 18-24 patients, with weekends up to 30 patients. - Support interdisciplinary team members including: - Physical Therapy - Social Services - Pharmacy - Transportation - No required weekend shifts; enjoy your weekends off! - Excellent opportunity for first-timers to gain experience in a Level II Trauma Center! - Guaranteed Hours: 72 hours per 2-week period - Must be able to include all requested time off at the time of submission — we respect your personal time! Qualifications: - Minimum of 1 year of experience in social services preferred. - Knowledge of community-based service programs including Medicaid and Medicare. - Ability to coordinate care effectively with internal and external teams. - Strong documentation skills to facilitate care coordination and discharge planning.
