Role Description This is a full-time, on-site Nurse Practitioner role located in Sycamore, IL. The Nurse Practitioner will be responsible for conducting patient assessments, diagnosing and treating a variety of medical conditions, creating care plans, and promoting preventive health management. Additional responsibilities include collaborating with physicians, educating patients on health maintenance, and ensuring compliance with healthcare regulations and best practices. Qualifications Strong clinical skills in patient assessment, diagnosis, and management of acute and chronic conditionsProficiency in developing and implementing individualized care plans and delivering preventive healthcare servicesExcellent communication, patient education, and interpersonal relationship-building abilitiesFamiliarity with Electronic Health Records (EHR) systems and documentation standardsCurrent licensure as a Nurse Practitioner (or eligibility for licensure) in the state of IllinoisMaster’s degree in Nursing or related field; national certification as a Nurse Practitioner preferredCompassionate, patient-focused, and committed to providing quality careExperience in internal medicine or adult primary care is a plus
