Medford, OregonWork Model: Onsite (Day Shift)Position Type: Full-TimeThe Physical Therapist provides skilled therapy services to patients with a wide range of diagnoses and functional limitations. This role focuses on patient evaluation, treatment planning, and rehabilitation to maximize independence and improve quality of life in a patient-centered care environment.Key ResponsibilitiesPerform patient evaluations and develop individualized treatment plans Provide physical therapy interventions to improve mobility and function Deliver care aligned with patient-specific needs and functional goals Collaborate with physicians and interdisciplinary teams Educate patients and families on rehabilitation and recovery plans Monitor patient progress and adjust treatment plans accordingly Maintain accurate clinical documentation and records Provide professional consultation and recommendations Ensure compliance with clinical standards and safety protocols Must-Have RequirementsDegree from an accredited Physical Therapy program:Bachelor's OR Master's OR Doctor of Physical Therapy (DPT) Active Oregon Physical Therapist License (or ability to obtain upon hire)BLS Certification (American Heart Association - upon hire)Strong clinical skills in evaluation and treatment planning Required SkillsExpertise in physical therapy techniques and rehabilitation practices Strong patient assessment and clinical decision-making skills Ability to work with diverse patient populations Excellent communication and patient education skills Strong documentation and organizational abilities Ability to work collaboratively in a multidisciplinary healthcare team Preferred / Nice-to-Have SkillsExperience in acute care or hospital-based therapy Specialized training in specific therapy areas (if applicable) Experience with complex or multi-diagnosis patients Core CompetenciesPatient-centered care Clinical excellence and accuracy Communication and collaboration Time management and organization Compassion and empathy EducationBachelor's / Master's / Doctorate (DPT) in Physical Therapy (Required)
