Licensed Practical Nurse

SUMMARYPerforms all dispensary and medical tasks necessary for the operation of a behavioral health clinic; assists in counseling groups; orients, trains and directs the work of other LPN's.DUTIESPerforms dispensary functions as assigned. Performs necessary injections.Performs basic nursing functions, such as taking blood pressure and completing medical history forms.Maintains accurate records on client dosage, attendance, and relevant side effects; maintains and updates the medical treatment follow-up file on all clients.May perform initial assessment and evaluation of clients.May participate in the development of treatment plans through data gathering and observations.May provide clinical outpatient and/or in-patient treatment, including crisis intervention, to individuals, families, and groups.May participate in case management services for clients, including development of treatment plans, follow-up, and closure.May perform urine monitoring as required.Assists physician in examinations, evaluations, and record keeping. May set up med clinics.Participates in team meetings; communicates information on client dosage, physical problems, and relevant behavior.Attends and participates in in-service training sessions. May present sessions as needed.Maintains dispensary, pharmacy, and examination room in acceptable sanitary condition.May make home, jail, and LARC visits as required.Coordinates with pharmacist as necessary for assembling doses for dispensing.May draw laboratory samples and facilitates follow up medical treatment.Serves as client advocate with community agencies as appropriate.May transport and assist clients for various testing and treatment in hospital or clinic/laboratory settingMay serve as staff liaison with hospitalized clientsMay provide med education for staff and clientsMay schedule nurse rotation on a monthly basisMay direct the work of other LPN's and may function as unit supervisor in his/her absence.Assists behavioral health providers as needed.Coordinate all care and services required to enhance and facilitate the optimal physical and behavioral health wellness of persons served within a trauma-informed care framework. This includes: 1) facilitating communication between medical and behavioral health practitioners/systems; 2) assisting with scheduling of and compliance with preventative and specialty physical health interventions; 3) integrating treatment interventions to address both the physical and behavioral health needs of the person/family concurrently; 4)assist persons/families served with education and resources outlining the interplay of physical and behavioral health issues on overall wellness and quality of life.An individual may qualify for the position if the individual is able to "perform safely" the essential functions of the position with or without reasonable accommodation.
