Evolve Therapy Services - Evolve Therapy Services is a therapy management organization hiring compassionate therapists for our partner located in Oakdale, NY!Job Position: Occupational Therapist for an In-house Skilled Nursing FacilityJob Type: Full Time or PRNJob Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.Job Functions:Conduct regular screening of residents to figure out the best intervention or treatment for each individual patientEvaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goalsInterpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team membersEstablish individual resident therapy schedules in cooperation with other team membersConduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy servicesPerform all other duties as assigned and as required to effectively treat patientsOversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followedEducation & Qualifications:Degree in Occupational TherapyMUST have an Active Occupational Therapy license in practicing state(s)Ongoing continuing education and professional development to maintain current licensure and certificationBenefits:Competitive Pay RatesMedical, Dental, VisionGrowth opportunities from within*Benefits based on Full Time Status
