PRN Speech Therapist

Job Description Job Summary: PRN Speech Therapist will provide direct patient care to home bound patients.Responsibilities And DutiesDevelops and revises plan of care in consultation with physician and other care-team members.Provides therapeutic treatments and evaluation of needs as applicable.Participates in clinical conferences and in-service programs.Conducts on-site supervisory visits of speech language pathologist assistants.Advises and consults with the family and other agency personnel as applicable.Prepares clinical and progress notes.Protects and honors patient and coworker confidentiality and right to privacy.Evaluates outcomes of care.Plans for discharge.Qualifications & Skills- Current Texas SLP License.- Current Certification in BSL/CPR- Valid Texas Driver's License and Automobile Insurance- Knowledge of WellSky EMR software is a plusSalary: Pay Per Visit including mileage reimbursement.
