A company is looking for a Mental Health Case Manager II-Therapist (Remote). Key Responsibilities Provides case management and therapy for patients and families to enhance psychosocial functioning Coordinates interdisciplinary care and oversees safe, comprehensive discharge planning Conducts behavioral health assessments, therapy, crisis intervention, and ongoing treatment planning with accurate documentation Required Qualifications Master's degree in Social Work, Counseling, or a related human-service field Licensure as a Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) in Missouri Minimum 2-3 years of Behavioral Health experience preferred Proficient computer skills
