Physical Therapist Location: Rutland, VT Agency: Cynet Health Pay: $2,646 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8am to 4pm with 2-3 weekend days Experience: Minimum 2 years Description: Actively monitors and amends patient visit schedules to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedules, including rescheduling visits and communicating recommended changes. Performs comprehensive assessments of assigned patients, including psychosocial, educational, safety, and risk factor parameters. Develops a plan of care to promote optimal functional independence and safety. Creates patient-specific goals and ensures that the plan of care reflects logical interventions and advancement toward these goals. Plans for and provides skilled physical therapy treatment including therapeutic techniques and patient/caregiver education. Assesses the patient's status and progress on each visit and ensures quality of care is documented. Coordinates patient care with other clinical team members, ensuring optimal patient care and communication. Supervises and directs clinical activities of support staff and students in accordance with applicable regulations and standards. Completes and coordinates required paperwork to ensure documentation reflects care provided and outcomes achieved. Ensures accurate charging and billing processes are executed, including supporting documents for insurance coverage. Participates in in-service training programs for various groups, including medical and nursing staff. Maintains and enhances personal knowledge and promotes standards in physical therapy. Proactively engages in the marketing and promotion of rehabilitation services, including community outreach. Carries out patient-related health assessments. Carries out various patient-related procedures, including wound care. Communicates changes in patient care schedules to relevant personnel in a timely manner. Completes required documentation in accordance with guidelines, regulations, and policies. Works collaboratively to maximize accuracy of clinical notes and documentation. Follows agency protocol for visit management processes. Accurately completes various required forms in a timely manner. Carries out visits in a time-effective manner and communicates information to the scheduler promptly. Assesses patients’ needs for assistance services and develops plans of care as necessary. Coordinates safe patient discharges and/or transfers and ensures appropriate paperwork is processed. Transmits patient incident reports to quality improvement staff in a timely manner.
