Nevada Licensed Discharge Planner (RN)

A company is looking for a Complex Discharge Planner (RN). Key Responsibilities Develops and coordinates complex discharge plans for high-risk members based on needs assessments Interacts with providers and community agencies to ensure safe and effective transition plans Educates members and their families on discharge plans and maintains documentation in health management systems Required Qualifications, Training, and Education Graduate from an Accredited School of Nursing or Bachelor's degree in Nursing 2 - 4 years of related experience, with 2+ years of acute care experience required Clinical knowledge of discharge planning competencies preferred Knowledge of Medicare and Medicaid regulations preferred RN - Registered Nurse - State Licensure and/or Compact State Licensure for the state of Nevada required
