Travel Licensed Practical Nurse
We are seeking a dedicated, adaptable, and compassionate Travel Licensed Practical Nurse to join our nursing and rehab team. The LPN will provide high-quality, patient-centered care to residents in skilled nursing and rehabilitation settings. This role requires strong clinical skills, excellent communication, and the ability to work independently as well as collaboratively with RNs, physicians, and interdisciplinary team members.
Key Responsibilities:
Deliver direct patient care to residents in accordance with physician orders, facility policies, and state regulations
Administer medications and treatments, document accurately, and monitor for adverse effects
Perform routine nursing duties such as vital signs, wound care, catheter care, denture care, and assistance with activities of daily living (ADLs)
Assess residents’ conditions, report changes to the supervising Registered Nurse (RN) or charge nurse, and participate in care planning
Assist with admissions, transfers, and discharges; complete required documentation and handoffs
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Maintain accurate and timely charting in electronic health records (EHR) and encounter documentation
Ensure infection control practices, safety protocols, and proper use of personal protective equipment (PPE)
Respond to emergencies, perform basic life support as trained, and escalate when necessary
Provide compassionate end-of-life care and support to residents and their families as appropriate
Support the facility’s quality assurance, safety, and regulatory compliance initiatives
Pay: $54.00 - $55.00 per hour
