Community Health Worker

Job Summary and Responsibilities As our Community Health Worker, you will be a cornerstone in driving healthy behaviors and improving health equity within our community, especially for high-risk and underserved populations. You will lead outreach efforts, implement vital health programs, and act as a crucial link between individuals and essential healthcare services in our Foundation Funded Employees department. Every day you will conduct outreach for various health initiatives, advising individuals and groups on critical issues like diabetes, obesity, prevention, immunization, and childcare. You will identify and contact members of high-risk groups (e.g., minorities, low-income, pregnant women), providing one-on-one coaching and cultural mediation related to health services. To be successful in this role, you will have a High School GED or High School Graduate qualification. You must be computer literate and possess strong customer service orientation with the ability to handle highly confidential information. Required skills include the ability to work independently, set priorities, manage work within timeframes, and communicate effectively, both verbally and in writing, with a positive mental attitude. * Empower participants to achieve their health goals through one-on-one coaching, adapting styles to individual learning preferences, culture, background, and beliefs. * Collaboratively identify unique risks, needs, and barriers related to healthy behaviors, chronic diseases (e.g., diabetes, heart disease, depression, arthritis) with participants. * Develop and evaluate personalized, participant-centered action plans, reviewing and adjusting them as needed to ensure ongoing progress and support. * Demonstrate active listening by creating a safe and supportive space for individuals to share challenges and concerns, showing empathy for their experiences. Job Requirements Required High School Graduate/GED Certified Community Health Worker training Complete Community Health Worker Certification within six (6) months of hire One (1) year of experience working with the community Experience working in a multicultural setting Electronic health record (EHR) experience Arizona Driver's License Preferred At least two (2) years of experience resourcing and engaging with non-profits or community-based organizations Previous experience in a clinical capacity or providing direct patient care (such as Certified Nursing Assistant or Medical Assistant) Bilingual proficiency (English/Spanish or English/any other language relevant to the community served) Where You'll Work About us: The Dignity Health East Valley Community Health department committed to elevating the health and well-being of our community.
