Job Title: LPN - Long Term Care (LTC)Location: New Lisbon, WIContract Duration: 13 WeeksShift:• PM Shift: 2:00 PM - 10:00 PM• Every Other Weekend RequiredStart Date: 06/01/2026Responsibilities:• Administer medications following the 5 Rights of Medication Administration• Monitor and report changes in resident condition to Charge Nurse and physicians• Communicate resident updates with family members as needed• Assist CNAs with call lights and resident care support• Report abuse, neglect, or misappropriation according to facility policy• Maintain accurate patient documentation• Support a safe and compassionate care environment• Perform additional nursing duties as assignedRequirements:• Active Wisconsin LPN License required• Current BLS/CPR Certification required• Long Term Care experience preferred• Minimum 1 year of related nursing experience preferred• Strong attention to detail and communication skills required• Ability to work rotating weekends and holiday coverage as neededFor more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
