Job Title: Site Physician AssistantDepartment: Clinical OperationsReports To: Chief Medical OfficerSchedule: Monday–Friday, 8:00 AM – 5:00 PM (40 hours/week)Position SummaryThe Site Physician Assistant provides high-quality, patient-centered care within a Diagnostic Treatment Center (DTC). This role includes primary care delivery, care coordination, and clinical leadership within an interdisciplinary team, with a focus on serving frail and elderly populations.Key ResponsibilitiesProvide primary care and manage a panel of patients in clinic and home settingsLead and support care delivery across the interdisciplinary teamParticipate in care planning, case conferences, and utilization reviewMonitor and improve quality, cost, and outcomes of careSupervise and support ambulatory medical servicesServe as a clinical resource for staff and provide education/trainingCollaborate with specialists and community providersSupport preventive care, outreach, and quality improvement initiativesQualificationsEducation & Certification:Graduate of an accredited Physician Assistant programNCCPA Board CertifiedActive, unrestricted NY State PA license and DEA registrationCurrent BLS certificationExperience:Background in Internal Medicine or Family Practice requiredExperience with geriatric or frail populations preferred (minimum 1 year or training provided)Experience in geriatrics or palliative care is a plusSkills:Strong clinical, communication, and teamwork skillsAbility to work in a multidisciplinary, patient-centered environmentAdditional RequirementsEligible to practice in New York StateMedicare/Medicaid provider eligibilityUp-to-date immunizations and medical clearancePhysical RequirementsAbility to stand, walk, lift up to 50 lbs, and perform clinical duties requiring mobility, manual dexterity, and clear communication.Job Title: Nurse Practitioner Supervisor (NPS)Schedule: Full-Time | Monday–Friday, 8:30 AM – 5 PMLocation: New York (onsite)OverviewWe are seeking an experienced Nurse Practitioner Supervisor to provide high-quality primary care while leading clinical coordination and care delivery for adult patients.
