In this role we are seeking an experienced Nurse Practitioner with 3 or more years of diabetes management experience.Here's what you'll be doing:We're looking for an experienced Nurse Practitioner who can work collaboratively with our Endocrinology team to deliver consistently positive outcomes for our diabetic patients. You will:Provide exceptional direct patient care, includingIn-person and virtual clinic visits, focusing on diabetes managementPatient phone calls and messages;Remote data analysis;Panel-level analysis to develop care plans for high priority patientsCall responsibilities, shared with other members of the clinical teamMaintain constructive, trust-based relationships with local primary care physicians and other members of the teamEngage with other members of the team in formal and informal ways to improve the health of specific patients and the overall panelWhat you need to be successful:Must be a graduate and certified from an accredited advance practice programThree or more years of clinical experience, with a demonstrated ability to work autonomously; capable of providing care without physician supervisionPrior work in diabetes is required; a huge plus if you are a CDCES or ADM board certifiedMust hold current, unrestricted State license or be eligible for licensureComfortable with Electronic Medical Record (EMR) systemsResponsibilities:The Diabetes Nurse Practitioner will provide specialty-level diabetes care in the outpatient clinical setting. Candidates with 3 years or more of experience in an acute care and/or physician practice setting with diabetes patients are strongly preferred. Candidates with strong internal medicine backgrounds and focus on endocrine/diabetes management will be considered. Position is outpatient based with a Monday-Friday schedule covering both our Newport News and Williamsburg locations.Candidate will have one year to acquire the Advanced Diabetes Management (BC-ADM) or Diabetes Care and Education Specialist certificationEducation:Licensure & CertificationCurrent Virginia Nurse Practitioner license or eligibility to obtain prior to start.National board certification through AANP or ANCC (FNP, AGNP, or ACNP).Valid DEA registration (or ability to obtain).BLS required; ACLS preferred depending on clinic or inpatient duties.Preferred: Certification as a Certified Diabetes Care and Education Specialist (CDCES) or willingness to obtain within a designated timeframe.Highlights:Competitive base pay with incentive bonusesComprehensive Health Benefits and Retirement PackageMalpractice CoverageCME AllowancePlease note: Visa sponsorship is not available for this position.
