CompanyBon Secours Home Care and Hospice by CompassusPosition SummaryThe Patient Care Coordinator's primary function is to schedule patient services and coordinate with staff.This is a full time role and will be office based, not remote. Position Specific ResponsibilitiesSchedules and appropriately documents patient schedules in a timely manner.Participates in coordinating care with management and patient interdisciplinary team.Provides effective communication to patients, staff members, other health care professionals, and referral sources.Responds to agency and patient needs in a professional and creative manner.Performs on-call responsibilities as assigned.Performs other duties as assigned.Education And/or ExperienceHigh school diploma or GED required.Experience with patient scheduling highly preferred.Previous home health scheduling experience preferred.Prior customer service preferred.At Compassus, including all Compassus affiliates, diversity, equity, and inclusion are fundamental to our Pillars of Success.
