Job Description: - Position: Transplant Social Worker - Specialty: Licensed Independent Clinical Social Worker (LICSW) - Shift: Day 5x8-Hour (08:30 - 17:00) - Years of Experience Required: 1 year(s) - Certifications Preferred: LICSW*, MSW Key Responsibilities: - Conduct biopsychosocial assessments and initial evaluations for pre/post liver transplant patients.- Provide crisis intervention and individualized patient/caregiver education.- Document care delivery, plans, goals, and patient/caregiver teaching/learning.- Participate in care conference activities and collaborate with interdisciplinary teams.- Verify caregivers over the phone as part of patient evaluations.- Work primarily in an outpatient setting with the potential for inpatient evaluations based on department needs. Patient Care: - Manage a caseload of 10 new patients per week.- Work with adult patient age groups.- Utilize knowledge of specialized areas such as drug/alcohol use and treatment options. Work Environment: - No weekends, holidays, or call requirements.- Supportive interdisciplinary environment with access to interpretation services, social services, and transportation. Skills and Knowledge: - Strong professional knowledge in advanced directives and protective services (Power of Attorney).- Experience in acute hospital settings (Emergency Department, ICU, Medical Surgical preferred).- ER or transplant experience is a plus! Additional Information: - First-time travelers accepted.- Candidates must be available for an interview prior to an offer.- Prefer LICSW certification; MSW required.
