This role involves providing mental health services to individuals in various community settings, including homes, schools, and other locations. The ideal candidate will have a strong understanding of mental health issues and be able to deliver care with empathy and professionalism.PRIMARY FUNCTIONS:Clinician will respond to mental health crises and provide immediate assessment, support and intervention.Clinicians are required to be skilled in the clinical triage and assessment of all emergency, urgent, and routine callsKnowledgeable about County resources with particular knowledge of the services of the AgencyBe able to assess needs of each individual served and intervene as appropriate to the situationBe able to coordinate outreach services, service planning, follow-up support and administrative back up as needed and clinically appropriateWork in compliance with MMHT, agency and regulatory procedural guidelinesConduct assessments and evaluations of clients' mental health needs in the community.Collaborate with other healthcare providers, social services, Law enforcements and community resources to coordinate care.Maintain accurate and up-to-date client records and documentation.Educate clients and their families about mental health conditions and coping strategies.Advocate for clients' needs and rights within the community and healthcare system.Coordinate follow up response to ensure stabilization.Perform other duties as assignedADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:• Assignments may be determined at a future dateQUALIFICATIONS AND ATTRIBUTES:Strong verbal and written communication skillsFlexibility with hours as may require some evenings and weekendsBilingual English and Spanish speaking a plus EDUCATION AND EXPERIENCE:Master's Degree AND NYS Licensure LCSW or LMHC1 to 3 years' experience preferably with individuals with mental health diagnosis PHYSICAL CHARACTERISTICS:These physical demands are representative of the physical requirements necessary for an employee to perform the job's essential functions successfully.
