Nurse Case ManagerOur client is seeking a Nurse Case Manager to support a coordinated, multidisciplinary approach to patient care across the healthcare continuum. This role is responsible for assessing, planning, coordinating, and evaluating patient care needs while serving as an advanced clinical resource for patients, families, physicians, and staff. The Nurse Case Manager plays a key role in facilitating safe, efficient, and effective care delivery.Education & ExperienceGraduation from an accredited school of nursingMinimum of three (3) years of nursing experience in an acute care hospital settingAt least three (3) years of experience in Case Management, Discharge Planning, or Utilization ReviewLicensure & CertificationActive Nevada Registered Nurse (RN) licenseBasic Life Support (BLS) certification from the American Heart Association (AHA)Additional & Preferred QualificationsRecent, documented acute care hospital experiencePediatric case management experience preferredRecent Emergency Department experience a plusRecent Utilization Review experience a plusOne or more of the following certifications or professional affiliations strongly preferred:Certified Case Manager (CCM)Accredited Case Manager (ACM)Membership in CCMC, ACCM, or ACMAKnowledgeCase management principles and nursing process standardsStandards of care and disease processes across multiple specialty areasPatient care planning, assessment, and evaluation techniquesNurse Practice Act and third-party reimbursement regulationsHospital safety practices, patient rights, and infection control policiesEmergency response procedures and age-specific patient care practicesSkills & AbilitiesManage patients with varying conditions and acuity levelsAnalyze and interpret medical records and clinical documentationApply regulatory and reimbursement guidelines effectivelyUse computers and healthcare software applications proficientlyCommunicate clearly and compassionately with diverse populations in high-stress environmentsBuild and maintain effective working relationships with interdisciplinary teamsUse equipment safely, efficiently, and effectively
