This role focuses on coordinating care, maintaining compliance, supporting staff, and ensuring participants receive high-quality services aligned with their individualized plans.✨ What You’ll Own: Demonstrate leadership in development and implementation of the agency-wide person-centered philosophy and practice. Role model and provide leadership and supervision to direct support staff. Maintain all participant records and services in accordance with applicable state, federal, and OPTIONS guidelines (such as annual planning meetings, quarterly reviews, data sheets, and intake information, etc.) for all assigned participants. Attend/coordinate/assist with annual CPOC/IPP meeting as needed in all programs. Ensure financial responsibility for funding and that services are implemented as stated in the CPOC/IPP. Ensure all funding source and licensing regulations are met. Work with participants, family members, case managers, DSW, and vocational staff to develop training goals and objectives that are appropriate for each individual as assigned and ensure all staff have knowledge to implement goals and objectives. Assist with representative payee paperwork/compliance for SIL participants ensuring financial stability for participant and abiding by all Social Security and OPTIONS regulations. Assist with participant bill paying and documentation. Assist with family monthly and weekly contacts as requested. Document all contacts in case notes. Ensure the participant satisfaction surveys are conducted and reported in an effective and timely manner. Provide 24-hour agency on-call support through participation in on-call rotation. Coordinate medical care and appointments to ensure all medical issues are addressed in a timely manner. Conduct comprehensive weekly home visit for participants as requested. Complete and document all required weekly contacts.
