The Behavioral Health Counselor will primarily support patients referred from the Belchertown Internal Medicine practice and provide virtual coverage to additional sites. The role is full‑time (40 hours per week) and hybrid, with onsite coverage 3‑4 days per week and remote coverage 1‑2 days per week.
Responsibilities
Provide short‑term, evidence‑based psychotherapy (individual, families, and groups) to patients of the primary care practice in‑person and virtually.
Develop comprehensive assessments and treatment plans for patients engaged in behavioral health treatment.
Offer a range of interventions to support primary care including crisis intervention, care coordination, information, referrals, and safety planning.
Assess new patients to aid in identifying the best treatment options.
Participate in formal and informal case reviews, seminars, and peer‑review meetings.
Support patients in accessing resources and managing care.
Collaborate closely with other Behavioral Health Team members, Primary Care, and Collaborative Care Teams to coordinate services and improve patient outcomes.
Education
Master's Degree in a related field of study required.
License
LMHC or LICSW in MA.
Experience
At least 3 years of related experience required.
