The Trinity Center is a120-bed skilled nursing facility which specializes in Alzheimer's care. If youare a skilled, compassionate nurse that is dedicated to making sure the bestquality of care is given, we want to talk to you!We are looking for an RN/LPN with prior experience in long-term orpost-acute care with solid clinical and leadership skills with the abilityto work independently to work two 16 hour shifts a week from 2p-10p/10p-6:30a and an 8 hour shift from 2p-10:30p EOW.JOB SUMMARYTheCharge Nurse will be responsible for planning, organizing, and carrying outnursing care of residents; orientation, instruction and supervision of CNA, CMAand resident care specialists; carry out or assign nursing duties to otherpersonnel and comply with established policies and procedures and performprofessional nursing duties within scope of licensureRESPONSIBILITIES:Keep DON/ADON informed of resident status and other pertinent mattersAdmit and orientate new resident and involves family with orientationCheck physician orders, transfer forms and pre-admission informationReview resident's medical and social historyComplete admission and documentation formsEnsure resident's supplies and medications are ordered and equipment is working properlyMaintain proper documentation of facility records: narcotic records, medical chart forms, weekly skin assessmentMaintain daily, weekly or monthly weight and vital signs and informs physician, DON, ADON and On Call Manager of any changesComplete incident reports, investigation reports and notify physician, family members, DON/ADON and On Call ManagerDischarge residents to community or other facilities, transfers residents to hospital, completes required forms and documentation, ensures resident is escorted to transportation and contact family prior to any transfers to hospitalREQUIREMENTSGraduate of an approvedschool of nursing for RN/LPNCurrent nursing license in good standing in the state of IowaCurrent CPR certificationpreferred or obtained within 30 days of hireOne year of experience in ageriatric setting preferredKnowledgeable with currentchanges in nursing practiceStrong computer skills including knowledge ofelectronic medical records requiredExcellent written and oralcommunication skillsWe offer a great totalcompensation package including:Competitive SalariesShift DifferentialMedical, Dental and VisionInsuranceCompany paid Long Term Disability and Life InsuranceVoluntary Short-termdisability, Hospital Indemnity, Critical Illness and Accident Insurance401k Retirement Plan with 4%company matchPTO plus THREE EXTRA DAYS for birthday, work anniversary and floating holidayReferral BonusesDiscount MealsUniform StipendContinuous Training andEducationTuition AssistanceBackgroundcheck, physical, drug screen and TB test required**LutherPark Community is an Equal Opportunity Employer**
