Role Description This full-time, on-site position as a Medical Social Worker is based in Nome, AK. The Medical Social Worker will be responsible for providing social work services to patients and their families, managing casework, coordinating discharge planning, and connecting individuals to appropriate social resources and services. The role involves collaborating with interdisciplinary teams to support patient care and ensuring patients receive holistic support within the healthcare system. Qualifications Proficiency in Medical Social Work and Social Work practicesExperience in Discharge Planning and Case ManagementStrong understanding of Social Services and their application in diverse healthcare settingsExcellent communication, organizational, and problem-solving skillsAbility to work empathetically and effectively with patients from diverse cultural and community backgroundsMaster's degree in Social Work (MSW) or related fieldState licensure or eligibility for licensure as a Medical Social Worker in AlaskaPrevious experience in rural or remote healthcare settings is a plus
