Work Schedule: Monday-Friday, daytime business hours (plus rotating on-call schedule)Anticipated Compensation Range: $80,000 - $85,000This is an on-site position with regular field visits for staff supervision and oversight of patient care.As a Registered Nurse (RN) - Case Management (Clinical Care Manager), you'll coordinate care and resources to support children and their families. This role blends compassionate, hands-on support for clients with leadership and guidance for our care teams. From conducting assessments and developing care plans to mentoring staff and ensuring compliance with high standards, you'll be at the heart of our mission to deliver exceptional pediatric home health care.In this role, you will:Manage a caseload of clients and coordinate services with the interdisciplinary team, patient/family, and referring agency, assuming responsibility for coordination of careComplete supervisory visits, comprehensive assessments, and reassessments in accordance with agency policy, applicable state and federal regulations, and established standards of practiceDevelop written plans of care to guide other health team members and document all findings, interventions, and progress in the clinical recordCommunicate plans and updates to the physician, nursing supervisor, and other staff through the care plan, written progress notes, and participation in care conferencesPrepare social histories to evaluate family dynamics, cultural considerations in relation to illness, financial impact, and other factors affecting the client's ability to meet care goalsProvide orientation and clinical training for new staff; support ongoing in-service education, continuing education, utilization review, documentation training, and Quality Assessment and Performance Improvement (QAPI) programsOffer direct guidance and timely responses to staff in resolving patient care concerns - demonstrate professionalism and serve as a positive role model for team membersEducate clients, families, and team members about how to access community resourcesActively participate in staff development activities, in-service meetings, case conferences, and other team or agency meetingsQualificationsRegistered Nurse licensed with the applicable State Board of NursingGraduate of an accredited nursing programCPR certificationCurrent valid Driver's License, good driving record, reliable transportation, and current auto liability insuranceProficiency with computer applications, including Microsoft Office and scheduling softwareStrong interpersonal, organizational, and communication abilitiesBilingual in Spanish preferred (not required)Why Angels of CareIn addition to our great benefits, we offer a fun and supportive culture rooted in our values of Heart, Advocacy, Love, Outreach, and Speed (HALOS).
