At Sarah’s Place, we are dedicated to making a meaningful impact in the lives of the people we serve every day.Role DescriptionThis is a part-time, on-site Social Worker or Social Services Coordinator position located in Haverhill, MA. The role involves providing essential support to participants and their caregivers/families, assessing individual and group needs, and coordinating necessary services to enhance quality of life. Responsibilities include developing and implementing care plans, liaising with other professionals and organizations, advocating for participants, and providing ongoing emotional and social support to participants. The Social Worker or Coordinator will also assist in monitoring participant progress and maintaining accurate documentation and records.QualificationsExperience in Social Work and Social Services to assess needs and implement support programsStrong Interpersonal Skills with the ability to communicate effectively with participants, families, and team membersExpertise in providing Social Support and coordinating community health services.Knowledge of relevant regulations and local resources for adult day health servicesAbility to manage multiple tasks, maintain detailed records, and adhere to confidentiality standardsBachelor’s degree in Social Work, Psychology, Sociology, or a related fieldLicensure or certification in social work is an assetPrevious experience in adult day health services, elder care, or a similar setting is a plusA genuine passion for helping others and fostering a supportive community
