Physical TherapistQualifications:Bachelor's (B.A., B.S.), Master's (M.A., M.S.) or Doctorate (DPT) degree in Physical TherapyPhysical Therapy board certification and licensure according to state regulationsHours:Full-time days: Monday-Friday 8:30 a.m. - 5:00 p.m.Pay period: Monday through SundayEmployment and Health Requirements:ResumeTwo recent employment referencesOnline verification of licensure or certification to include expiration date and any disciplinary actions within 30 days of start dateBLS - Copy of both sides of the card – through American Heart AssociationImmunizations to include:MMR (Mumps, Rubeola & Rubella) either 2 doses or Positive TitersVaricella-positive titer or 2 Varicella VaccinesPolioHepatitis B Series, titer or signed declinationCurrent TDaP – Tetanus, Diphtheria and Pertussis (dated within 10 years of start)COVID-19 vaccineProof of Flu vaccination during Flu Season (Oct 1st – March 31st)Negative 2-step Mantoux within 1 year of start or QuantiFERON Gold if Positive, must provide proof of positive & chest x-ray resultsProof of physical within 1 year of start date10 Panel Drug Screen within 1 month of start dateRespiratory Fit Test within 1 year of start date (we can provide RFT if not available)Criminal Background (All states where they hold a professional license and have lived/worked in the past 7 years) within 30 days of start dateNational &/or State Sex Offender GSA(EPLS) Sam.gov within 30 days of start dateOIG-Date within 30 days of start dateIowa Mandatory Reporter Certificates (Child Abuse & Dependent Adult Abuse)Completion of CareLearning education modules (further instructions will be provided by JCHC) Mandatory Reporter will be added to modules if the candidate is not certified. Must have completed prior to start.
