The ideal candidate brings existing provider relationships, strong market knowledge, and the ability to independently grow a hospice census.What You'll DoLeverage existing relationships with physicians, hospitals, SNFs, and community partners to drive referralsActively grow hospice admissions through strategic business development effortsPosition Choice as the preferred hospice provider in the marketEducate referral sources on hospice eligibility, services, and benefitsCollaborate with clinical teams to ensure seamless patient transitionsExecute targeted outreach and community engagement strategiesConsistently meet or exceed monthly admission goalsWhat We're Looking ForRequired: Proven experience in hospice or home health business development/liaison roleDemonstrated success in meeting or exceeding admission/census goalsEstablished relationships within the local healthcare community (strongly preferred)Deep understanding of hospice services, regulations, and referral processesHighly self-directed, with the ability to manage a territory independentlyValid driver's license and auto insuranceHomeCare HomeBase experience (preferred)Medical, Dental, and Vision Insurance401(k) with immediate vesting (100% match on first 3%)HSA, Life Insurance, Short/Long-Term DisabilityPTO & Paid HolidaysCompany vehicle (after 90 days, if eligible)Why This Role Stands OutThis role is designed for a seasoned hospice liaison who wants the autonomy to grow a territory and make a direct impact on patients and families through increased access to end-of-life care.J-18808-Ljbffr
