Develop and implement person-centered treatment and discharge plans in collaboration with seriously mentally ill consumers and other member of the interdisciplinary treatment team. Serve as link between community agencies and others that support consumer's recovery and return to community. Minimum Qualifications Minimum of a Bachelor's in Social Work (BSW) is required. Experience providing case management and discharge planning services to the mentally ill population. Valid driver's license. Additional Considerations Experience with community resources for mentally ill population.
