The RN will manage a variety of cases, including chronic and acute conditions, while ensuring proper documentation and adherence to care standards. This role requires strong clinical skills, independence, and the ability to adapt to different patient environments. Key ResponsibilitiesProvide skilled nursing care to Home Health and Hospice patientsPerform patient assessments and develop individualized care plansHandle admissions (SOC), recertifications, and resumptions of careAdminister medications, IV therapy, and wound care treatmentsManage chronic conditions such as CHF, diabetes, and renal diseaseEnsure accurate and timely OASIS documentationEducate patients and families on care plans and disease managementMaintain infection control standards in home settingsParticipate in on-call rotation (evenings and weekends)Patient LoadAverage of 6 points per day (e.g., 1 admission + 3–4 visits)Required QualificationsMinimum 1 year of RN experienceRecent Home Health experience (within last 2 years)Strong OASIS documentation proficiencyExperience with wound care and infection controlActive RN license (required at submission)BLS certification (AHA) requiredAbout LanceSoftEstablished in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
