In this role, you will provide high-quality, individualized therapy services to residents in our subacute rehabilitation unit, helping them achieve their highest level of function and independence. Flexible scheduling is available, with both full-time and per diem opportunities.Key ResponsibilitiesEvaluate, plan, and implement individualized physical therapy programs for residentsProvide therapeutic interventions including mobility training, balance exercises, strengthening, and gait trainingMonitor and document resident progress, updating care plans as appropriateCollaborate with physicians, nursing staff, occupational therapy, and speech therapy to ensure coordinated careEducate residents and families on exercises, adaptive techniques, and safe mobilityParticipate in team meetings and quality improvement initiativesMaintain compliance with federal, state, and facility regulationsQualificationsLicensed Physical Therapist in the state of IowaMinimum of 1 year of experience in a skilled nursing or subacute rehabilitation setting preferred (new graduates welcome)Strong clinical reasoning, communication, and documentation skillsAbility to work independently and as part of a multidisciplinary teamCPR certification required or willingness to obtainWork ScheduleFull-Time: Monday–Friday, standard business hours (flexibility may be required)Per Diem: Flexible scheduling based on facility needsWhat We OfferPremium pay rates up to $75/hourCompetitive salary and per diem ratesHealth, dental, and vision benefits (full-time)Paid time off (full-time)Continuing education opportunitiesSupportive and collaborative work environment
